Northwell Health in Suffolk, VA is seeking a Community Health Worker liaison who anchors grant support for vulnerable community members, conducts assessments and in-home visits, and coordinates referrals to social services. The position includes mentoring Community Health Workers, supervising member care to improve outcomes, and collaborating with the care team to deliver health education and ensure access

Qualifications

  • High School Diploma or equivalent required.
  • 1-3 years of relevant experience, required.

  • Acts as a resource for CHW and members
  • Provides support and health education about material and general health care and community resources.
  • Conducts one-on-one health education, community outreach, regular in-home visits and service coordination.
  • Provides various supports, services, entitlements and benefits for members.
  • Supports members in developing health management care plans.
  • Empowers members by promoting effective approaches, problem solving and lifestyle changes.
  • Works to establish positive and supportive relationships with a diverse group of members.
  • Delivers coaching and mentorship on chronic health conditions and self-care.
  • Enters client data into software daily.
  • Advocates for members and families and collaborates with the care team as needed.
  • Performs all related duties as required under ADA.

Works as a liaison to grant support to the vulnerable members of the community, addressing material and health challenges. Maintains a caseload of members and provides referrals to various support services by conducting assessments and in home visits. Serves as a role model and will identify and initiate referrals to social service programs, serve as main point of contact for the member and support the implementation of various care activities.
