Job summary

Northwell Health is seeking a Community Health Worker Supervisor to liaise with vulnerable community members, conduct assessments, provide in-home visits, and connect clients with social services and health resources.

You will mentor Community Health Workers, coordinate care activities, maintain caseloads, and advocate for members within a multidisciplinary team to improve health outcomes and access to benefits.

Qualifications

• High School Diploma or equivalent required.
• 1-3 years of relevant experience, required.

Responsibilities

• Serves as a mentor for Community Health Workers.
• Provides oversight in member care to improve health outcomes.
• Acts as a resource for CHWs and members.
• Offers health education about materials and general care resources.
• Conducts one-on-one health education, community outreach, regular in-home visits and service coordination.
• Provides various supports, services, entitlements and benefits for members.
• Supports members in developing health management care plans.
• Empowers members by promoting effective approaches, problem solving and lifestyle changes.
• Works to establish positive and supportive relationships with a diverse group of members.
• Delivers coaching and mentorship on chronic health conditions and self-care.
• Enters client data into software on a daily basis.
• Advocates for members and their families and collaborates with the care team as needed.

Serves as a role model and will identify and initiate referrals to social service programs, serve as main point of contact for the member and support the implementation of various care activities.
