Northwell Health in New York is seeking a Community Health Worker liaison to grant support to vulnerable community members, conduct assessments, and perform in-home visits while coordinating referrals to social services. Works as a liaison to grant support to the vulnerable members of the community, addressing material and health challenges. Maintains a caseload of members and provides referrals to various support services by conducting assessments and in home visits. Serves as a role model and will identify and initiate referrals to social service programs, serve as main point of contact for the member and support the implementation of various care activities.
