  • 1.Serves as a mentor for Community Health Workers
  • 2.Provides oversight in care of members to improve health outcomes and promote well-being
  • 3.Acts a resource for CHW and members
  • 4.Provides support and basic health and education about material and general health care and community resources.
  • 5.Conducts one-on-one health education, community outreach, regular in-home visits and service coordination.
  • 6.Provides various supports, services, entitlements and benefits for members.
  • 7.Supports members in developing health management care plans.
  • 8.Empowers members by promoting effective approaches, problem solving and lifestyle changes.
  • 9.Works to establish positive and support relationships and effective works with a diverse group of members.
  • 10.Delivers coaching and mentorship on chronic health conditions and self-care.
  • 11.Enters client data into software on a daily basis.
  • 12.Advocates for members and their families and collaborates with the care team as needed.

Maintains a caseload of members and provides referrals to various support services by conducting assessments and in home visits. Serves as a role model and will identify and initiate referrals to social service programs, serve as main point of contact for the member and support the implementation of various care activities.
