The mission of Charles River Community Health (CRCH)is to partner with individuals and families so they can thrive and lead healthier lives by delivering the comprehensive, integrated, and equitable primary healthcare that matters most to them. The Community Health Worker (Spanish) will serve as a frontline public health worker and medical interpreter. As part of a dynamic team at CRCH, ahealthcare practice providing compassionate care to underserved communities, the Community Health Worker (CHW) will serve as a frontline public health worker and interrupter.

The CHW is a trusted member of the community and has a strong understanding of the Spanish-speaking communities. The CHW serves as a liaison between the health provider and social services and the community/patients. As such, the CHW will facilitate access to services and improve the quality and cultural competence of service delivery. If you have a calming presence, excel at understanding patient needs, and are willing to work on educating patients about the importance of health screenings, we would like to have you as part of our team. The CHW will work as a team member at our health center and will interact with patients and staff who are racially, ethnically, and culturally diverse, so being culturally competent is required.

YOUR RESPONSIBILITIES

  • Conduct accurate medical and general interpretation for patient care staff, patients, and their families.
  • Translate documentation, posters, and flyers.
  • Establish trusting relationships with patients and their families while providing general support and encouragement.
  • Provide ongoing follow up, basic motivational interviewing and goal setting with patients/families.
  • Provide referral for services to community agencies as appropriate.
  • Remain knowledgeable about community resources appropriate to needs of patient/family.
  • Enable and improves culturally and linguistically competent services and care by making efforts to understand the community culture and resources.
  • Conduct outreach, implements programs in community that promotes, maintains and improves community health needs and provides resources.
  • Collect data to help identify community health needs.
  • Advise community groups on chronic disease and ways of prevention
  • Provide informal counseling as needed.
  • Perform other duties as assigned by Community Health Worker Lead or designee.

YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

  • Bilingual Spanish/English speaking required. Must be proficient in English and fluent in Spanish.
  • High school diploma or GED, at least, required.
  • Must have proof of completing a minimum of 40 hours of medical interpretation training. Or be willing to obtain training on the job and obtain certification within 1 year of employment.
  • At least 1 year of experience working within a healthcare setting
  • Certification in Community Health Worker or Medical Interpreter preferred but not required.
  • Must have knowledge of computer systems such as Word and Excel.
  • Strong interpersonal, organizational, and time management skills required.
  • Knowledge of both community and healthcare system resources.
  • Experience serving the population(s) that will be referred for patient navigation services preferred.
  • Excellent written and oral communication skills required.
  • Must have a willingness to work flexible hours to meet the organization's needs/demands, including work on evening and/or Saturdays 8:30a-1:00p (otherwise, there is some flexibility with the schedule).
  • Must be able to work in either site or between sites (Brighton/Waltham) as needed.
  • Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
  • Must have experience in diverse cultures, with strong commitment to promoting Diversity, Equity, and Inclusion and reducing inequities.
  • Must believe in the work we do at CRCH, with a strong passion to serve underserved populations in diverse settings.
