Summary

The CHW I acts as a liaison between the health system and the communities they serve. Develops and maintains relationships with community members, health and human services providers and advocacy organizations and agency partners in communities served by CHA. The CHW I builds an active, coordinated referral network that supports collaborative, working relationships between community agencies, patients, and CHA. Conducts outreach and engagement in the community at events, health fairs, workshops, conferences, and other community/public forums. The CHW provides care coordination, health education, advocacy and access to culturally appropriate social or health services to promote health in a community.

Responsibilities

  • Establishes trust and communication with community members and provides support and access to social and health resources.
  • Conducts SDOH screening and, when appropriate, provides and documents referrals along with additional support to meet the identified gaps
  • Helps community members connect to and provides referrals to social and health resources in a culturally and linguistically appropriate way.
  • Does follow up with community members or patients, as appropriate, to ensure connection to services and records interactions in the EMR or database.
  • Develops a comprehensive listing, initiates, contacts, and maintains working relationships with health and human service providers, advocacy organizations and other groups in order to facilitate and carry out referrals between eligible populations and CHA programs
  • Contributes to and updates a database of community contacts made and outreach activities conducted within the service area.
  • Facilitates or conducts periodic in-services for staff and/or clients at local health and human service providers and other community agencies
  • Supports and implements division work plan as guide for targeted outreach activities
  • Provides program, education and outreach materials to local agencies and advocacy groups
  • In coordination with the manager, represents CHA on committees or public health initiatives, as well as other community activities.
  • Represents CHA at community health fairs, workshops, and other public forums.
  • Submits all documentation and reports as requested by supervisor
  • Participates in department sponsored educational sessions
  • Participates in staff meetings
  • Develops appropriate education and outreach materials on various topics
  • Other duties as assigned

Other Information

The Qualifications Required At Hire Are

  • High School, GED or Associate’s Degree graduate, knowledge of community and neighborhoods served by CHA and fluency in English
  • Well-organized, systematic approach to work.
  • Good communication skills in oral and written English
  • Sensitivity to the needs of the participant population

Certifications

  • CHW certification encouraged

Language

  • Bi-lingual/ BI-cultural Bilingual preferred (specific bilingual capacity needed as appropriate to the community served by CHA)

Previous Work Experience

  • Two years experience in community education and outreach or health and human services preferred.
  • Experienced working with persons of different cultures and economic status desired.
  • Basic computer skills and knowledge of google suites
  • Experienced in coalition and committee consensus building, community advocacy and liaison work.
  • Able to interact and collaborate successfully with patients, community members, and professional staff is essential.
  • Good oral and written skills.
