Job summary

BJC HealthCare in St. Louis is seeking a Community Health Worker to improve preventive health outcomes by partnering with an interdisciplinary care team and community resources.

You will identify gaps in care and help clients access services, document plans, and follow up with patients in person or by phone. Responsibilities

• Work daily as a community liaison to identify gaps in care by assisting clients in accessing resources to remove barriers related to social determinants of health.
• Provide referrals for appropriate services by assisting in assessments/screenings or conducting follow-up care with patients in person, via phone, home visits and/or visits to other settings where clients are accessible.
• Establish and maintain trusting relationships with clients daily to effectively work with an inter-disciplinary team, internal departments, and community organizations.
• Document and track ongoing activities, care plans, and results in an effective manner while adhering to policies and procedures.
• Continuously communicate and expand knowledge of community resources and programs to reduce barriers between clients and institutions.
• This is a safety-sensitive position and requires the ability to work in a constant state of alertness.
