Job summary

The Pennyrile District Health Department is seeking a grant-funded Community Health Worker to provide one-on-one support through care coordination, barrier navigation, and connections to housing, treatment, legal assistance, and employment services.

The CHW acts as an advocate and guide, helping clients navigate challenges and pursue recovery and wellness goals, visiting clients in appropriate settings and assisting with appointments and transportation.

Mission Statement

The Kentucky Department for Public Health (DPH) is responsible for developing and operating state public health programs and activities for the citizens of Kentucky.
