Job summary

Kentucky Public Health in Pennyrile District is seeking a grant-funded Community Health Worker to provide one-on-one support through care coordination, barrier navigation, and connections to resources such as housing, treatment, legal assistance, and employment services.

The CHW will advocate for clients, assist with transportation and appointments, and help individuals navigate services to achieve recovery and wellness goals.

Qualifications

• High School Diploma or GED required.
• Experience as CHW not mandatory but helpful.
• Valid driver’s license required.
• Must pass background check and drug screen.

Responsibilities
