Job summary

Malph is seeking a dedicated health navigator to work in Mecosta County, Michigan. This in-person role involves engaging with participants one-on-one to assess medical, behavioral, and social health needs, and to develop comprehensive care plans.

You will connect clients to community resources, provide Medicaid outreach in office, clinic, and community settings, and act as a liaison to reduce barriers to health and wellbeing in the local population.

• Collects and records information on barriers to good health; liaises to identify and navigate community resources using an evidence-based model.
• Provides navigation services and Medicaid outreach in office, clinic and community settings.
• In-person position based in the Mecosta office and covers Mecosta County.

Job description

General Responsibilities

• Under general supervision, engages one on one with participants from a variety of backgrounds to identify their medical, behavioral, and/or social health and wellbeing needs; works with the participant to develop a care plan; and connects the participant to community resources to improve their health.
• Obtains and records information contributing to the participant’s barriers to good health and acts as a liaison to identify and navigate community resources using an evidence-based model to reduce barriers to health and wellbeing.
• Provides navigation services and Medicaid outreach in office, clinic, and community settings.
