Position Summary

  • Better Health is hiring for multiple openings across Kentucky.
  • Case Management Coordinator (CMC) utilizes critical thinking and professional judgment to support the case management process, aiming to improve healthcare outcomes by providing advocacy, collaboration, coordination, support, and education for members through care management tools and resources.
  • This is a telework position.
  • Qualified candidates must reside in Kentucky and support members throughout the state.
  • Flexibility to work beyond core hours of Monday-Friday, 8am-5pm EST, is required to serve children and families after school or work hours.

Duties

  • Evaluation of Members: Conduct assessments and review data to recommend approaches for resolving care needs, evaluate benefit plans, and identify high-risk factors to implement early interventions.
  • Coordinate and monitor wellness care plan activities to meet member needs.
  • Enhancement of Medical Appropriateness and Quality of Care: Consult with multidisciplinary teams, present cases at conferences, escalate quality issues, and negotiate services to meet healthcare needs.
  • Use motivational interviewing to promote member engagement and healthy lifestyle changes.
  • Provide coaching and support to empower members in healthcare decisions and participation with providers.
  • Monitoring, Evaluation, and Documentation: Ensure compliance with regulatory guidelines and company policies in case management processes.

Experience

  • Minimum 6 months experience in Foster Care, juvenile justice systems, Adoption Assistance, Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention, or related practices applicable to Kentucky SKY populations.
  • Minimum 2 years in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling.
  • Minimum 1 year experience with MS Office applications.
  • Must reside in Kentucky.

Preferred

  • 3+ years' experience in behavioral health or social services.
  • 1+ MCO experience preferred.

Education

  • Bachelor’s degree or non-licensed master’s in behavioral health or human services field required.
