Job summary

Your Health in Myrtle Beach, SC is seeking a dedicated Community Health Worker to serve patients in the [LOCATION] area. The CHW links patients with healthcare services, facilitates home and ALF visits, and ensures access to medications and DME.

You must have reliable transportation as travel is daily and shifts run 7:00 AM–7:00 PM. Responsibilities include coordinating visits with care teams, delivering medications, enabling telehealth and RPM, and assisting with DME.

Qualifications

• Must have relevant healthcare licensure or certification in good standing.
• Experience with DME delivery or logistics preferred.
• Willingness to travel daily and drive a vehicle.

Responsibilities

• Coordinate visits with care team to ensure coordinated patient care.
• Deliver medications and review instructions at patient homes.
• Arrange telehealth visits between patients and providers.
• Set up RPM equipment training and provide ongoing support.
• Deliver DME and ensure proper use and training.
• Conduct venipuncture and collect samples when needed.
