Job summary

Family Service of Rhode Island seeks a Community Health Worker to join a multidisciplinary team in Providence, RI. The role supports referrals to the Caring Dads program, delivering extensive case management to referred fathers and coordinating intake and screenings.

The CHW will co-facilitate a 17-week group intervention, connect with mothers, identify needs, and ensure referrals to appropriate services, while maintaining flexible scheduling and safety requirements.

Qualifications

• High School Diploma or Equivalent is required, degrees preferred.
• Associates or Bachelor's degree preferred.
• Community Health Worker Certification preferred.
• Bilingual in Spanish or Portuguese preferred.

Responsibilities

• Engage with referred fathers and provide extensive case management.
• Assist with completing intake documents and conduct health screenings.
• Identify needs and ensure referrals to matched services.
• Co-facilitate 17-week group sessions for referred fathers.
• Serve as the mother contact to discuss Caring Dads components and coordinate referrals.
• Provide ongoing support and monitoring as needed.
