Qualifications

Responsibilities

• Identify and enroll individuals needing healthcare services.
• Interview and enroll patients and families for education and care.
• Provide home-based support and guidance to clients.
• Assist clients in accessing healthcare and social services.
• Advocate for families and coordinate with care teams.
• Develop and implement individualized intervention plans with Care Coordinator.
• Follow up on referrals and missed appointments.
• Maintain accurate records of visits, referrals and education provided.
