Job summary

Family Service of RI in Providence, RI is seeking a Community Health Worker to join a multidisciplinary team. The CHW will provide intake assistance, case management, referrals, and social determinants of health screening to referred fathers in the Caring Dads program.

The role includes co-facilitating a 17-week group for fathers, connecting with mothers, and coordinating services across agency sites, community locations, and virtual options, with some evenings and travel.

Qualifications

  • Associates or Bachelor’s degree preferred, HS diploma acceptable.
  • Community Health Worker Certification preferred.
  • Ability to meet certification requirements within the first 12 months of employment.

Responsibilities

  • Work as part of a multidisciplinary team to support referrals to the Caring Dads program.
  • Provide extensive Case Management for referred Fathers and complete Intake documents.
  • Conduct Social Determinants of Health and other screenings; identify needs and ensure referrals to matched services.
  • Co-facilitate 17-week group intervention meetings for referred Fathers to promote healthy family interaction and non-abusive co-parenting.
  • Serve as the Mother Contact to discuss Caring Dads components, complete screenings, identify needs and coordinate referrals and ongoing support.

FSRI is always looking for candidates that want to make a positive impact on the community!Position Summary: The CHW will work as part of a multidisciplinary team to support referrals to the Caring Dads program. CHW is responsible for engaging and providing extensive Case Management for referred Fathers; assist with completing Intake documents; Social Determinants of Health and other screening tools; identifying areas of need; ensuring referrals are made to appropriately matched services. CHW will co-facilitate specialized 17-week group intervention meetings for referred Fathers aimed to promote healthy family interaction, active listening, child centered parenting, and foster non-abusive, co-parenting relationships. Additionally, the CHW will serve as the “Mother Contact” connecting with the identified partner to discuss Caring Dads program components, complete a Social Determinants of Health screening as needed, identify needs and ensure referrals are made to the appropriately matched services and provide ongoing support and monitoring as needed.Qualifications:Associates or Bachelor’s degree preferred, High School Diploma or Equivalent required.Community Health Worker Certification preferredAbility to meet all requirements to be certified as a Community Health Worker within the first 12 months of employmentBilingual in Spanish or Portuguese preferredStrong interpersonal and social skills with an ability to collaborate with a variety of community based organizations, coalitions and stakeholdersPrior experience in group facilitation, victim advocacy, domestic violence offenders and/or crisis intervention preferredCommitment and sensitivity to issues related to the needs of parents and children, as survivors of Domestic ViolenceAbility to work independently and as an interdependent team memberAbility to provide services at agency sites, community locations, and virtually as requiredAbility to have a flexible schedule to include some eveningsMust pass RI BCI, National Fingerprint & DCYF CANTS checkValid driver’s license and reliable transportationPhysical Requirements:This position requires agency and community visits, employees in this position must have the ability to:Travel to and from community locations and agency sites, which could include using walkways, stairs and/or elevators.Ability to lift up to 20lbs.Ability to communicate effectively.Don’t meet every single requirement?
