Job summary

VIVA HEALTH in Birmingham, AL is seeking a Community Health Worker to support patients through the ACHN and Medicare teams. You will promote disease self-management, share information on medical and community services, and help carry out the plan of care with Care Coordinators and Case Managers.

Travel within the service area may be required; you should have good communication skills, empathy, and the ability to work across diverse community settings to improve recipients' health outcomes.

Qualifications

• High School Diploma or GED required.
• Strong interview and telephone skills; caring, assertive and tactful.
• Ability to understand health-related service delivery systems.
• Excellent communication and relationship skills.
• Ability to analyze diverse environmental factors affecting members’ well-being.
• Strong organization and time management.
• Flexible, adaptable and able to work in varied community settings.

Responsibilities

• Assist Care Coordinators and Case Managers in care management services.
• Promote disease self-management using approved education tools.
• Provide patients with information on medical and community services.
• Assist with carrying out the established plan of care as directed.
• May assist with Health Risk Assessments and referral generation.
• Travel to locations within the service area as needed.
