Job summary

VIVA HEALTH in Auburn, AL is seeking a Case Manager to assess the social, emotional and environmental needs of ACHN patients to improve medical compliance and health outcomes. You will identify barriers to care, such as transportation or literacy, and ensure services are delivered with continuity of care.

This role involves analyzing home and community contexts, developing patient-centered care plans, and coordinating services primarily in community and home settings.

Qualifications

• Licensed BSW, BSN, or ADN.
• Current RN or SW license in good standing with the Alabama board.
• Valid driver's license in good standing.
• One to three years in a community-based clinical role.
• Licensed MSW or BSN (preferred).
• Experience in case management, human services, public health, or with underinsured population (preferred).
• Certified Case Manager (preferred).

Responsibilities

• Assess the social, emotional and environmental needs of ACHN patients.
• Identify barriers to medical compliance and ensure services are delivered with continuity of care.
• Analyze home and community contexts to develop patient-centered care plans.
• Coordinate services in community and home settings; travel as needed.
