Job summary

Clinica Family Health & Wellness is seeking a Community Health Worker to engage underserved communities, educate on behavioral health resources, and connect people to available services in Boulder County. You will build trusted relationships through culturally competent outreach and report to the Community Outreach Manager.

This role requires in-community work schedules, a valid Colorado driver’s license, and a commitment to reducing health disparities through direct connection to care.

Qualifications

• High School Diploma, GED, or equivalent.
• 21+ years old with a safe Colorado driver's license.
• Reliable transportation to connect with clients.
• Connected in the community to link clients to behavioral health services.
• Lived experience in the behavioral health care system.

Responsibilities

Job description

As our Community Health Worker, you will work directly in the community with underserved populations to guide and increase community education that would support community members to connect to our services for their behavioral health.

You will work in Boulder County to provide education on mental health facts, available services and create avenue(s) to connect with services. You will be the trusted partner in the community building relationships based on your shared life experiences. You will report to the Community Outreach Manager.

Job Profile:

• Are you able to connect with people to learn about their barriers and be able to guide them to available resources?
• Are you motivated to reduce health disparities in underserved populations by identifying and understanding barriers they can connect to

What We Need:
