Job summary

Pacific Health Group seeks a Housing Navigator/Case Manager to support CalAIM Community Supports Housing programs, including Housing Transition Navigation, Housing Tenancy & Sustaining Services, and Housing Deposit assistance. You will conduct outreach, assessments, in-person visits, and follow-ups to develop individualized housing care plans and ensure housing stability.

The role requires strong knowledge of local housing resources, Medi-Cal/CalAIM familiarity, and excellent communication.

Qualifications

  • Strong understanding of the housing system and local housing resources or transitional housing support workflows (Medi-Cal, CalAIM, HUD programs).
  • 3-5 years in case management, social services, or healthcare.
  • Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
  • Strong understanding of healthcare systems, managed care, and community resources.
  • Excellent communication, empathy, and cultural competence.
  • Strong organizational and time management skills.
  • Proficient in technology, including case management systems, EHR platforms, and documentation tools.
  • Ability to document in real-time and manage member data accurately.

  • Coordinate housing applications, Housing Deposit Services packages, landlord allocations, appointments, services, and long-term housing support.
  • Monitor housing stability, address barriers, and ensure continuity of care.
  • Provide support by phone, video, and in-person visits.
  • Represent Pacific Health Group at outreach events and local initiatives.
  • Navigate multiple platforms and complete real-time chart notes and clinical documentation.

Job description

  • Support members participating in CalAIM Community Supports Housing programs, including Housing Transition Navigation Services, Housing Tenancy & Sustaining Services, and Housing Deposit assistance
  • Conduct outreach, intake assessments, comprehensive member assessments, in-person visits, and follow-ups
  • Provide person-centered housing care management and develop individualized housing care plans
  • Build partnerships with housing providers, shelters, landlords, housing authorities, property managers, healthcare providers, and community organizations
  • Coordinate housing applications, Housing Deposit Services packages, landlord allocations, appointments, services, and long-term housing support
  • Support housing transitions, tenancy retention, advocacy, landlord engagement, and connections to healthcare, behavioral health, and community resources
  • Monitor housing stability, address barriers, and ensure continuity of care
  • Provide support by phone, video, and in-person visits
  • Represent Pacific Health Group at outreach events and local initiatives
  • Navigate multiple platforms and complete real-time chart notes and clinical documentation
  • Maintain accurate documentation, track outcomes, and ensure compliance with Medi-Cal, CalAIM, and Community Supports requirements
  • Collaborate with interdisciplinary teams, healthcare providers, and payers
  • Provide cross-coverage, share housing resources, assist with Lead Care Manager shadowing sessions, and promote team best practices
  • Identify service gaps and trends and collaborate with leadership to improve workflows
Requirements
  • Strong understanding of the housing system and demonstrated working knowledge of local housing resources, housing navigation, community-based support programs, or transitional housing support workflows (such as Medi-Cal, CalAIM, Community Supports, or HUD programs)
  • 3-5 years in case management, social services, or healthcare
  • Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred)
  • Strong understanding of healthcare systems, managed care, and community resources
  • Excellent communication, empathy, and cultural competence
  • Strong organizational and time management skills
  • Proficient in technology, including case management systems, EHR platforms, and documentation tools
  • Ability to document in real-time and manage member data accurately
  • Comfortable navigating multiple systems and communication platforms
  • Ability to travel locally within Mendocino County
  • Valid California Driver's License and reliable transportation
  • Ability to work in a hybrid, field-based environment
  • Successful completion of a background check
