Community Health Worker - Galia County

We are currently seeking a Community Health Worker in the Clinical department to conduct program assessments using established tools and processes. This is a home-based position that requires independent management of a caseload.

The Position

CareStar is looking for motivated individuals who:

1. Hold a Bachelor's Degree
2. Have 12 months of experience in home and community-based services within the past 5 years
3. Possess knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs (desirable)
4. Adhere to the CareStar Rules in performing job responsibilities
5. Understand and comply with CareStar Policies and Procedures
6. Maintain confidentiality related to patient information; unlawful disclosures outside the proper course of duty will be treated as serious misconduct
7. Follow the Acceptable Use Policy when using CareStar's information systems
