Community Health Worker - Lawrence County

This is a home-based position that requires managing a caseload independently.

The Position

CareStar is looking for motivated individuals who:

• Have a Bachelor's Degree
• Have 12 months of experience in home and community-based services within the past 5 years
• Possess knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs (desirable)
• Adhere to the CareStar Rules and Policies
• Maintain confidentiality of patient information and follow the Acceptable Use Policy
