Horizon Health in Mission, SD is seeking a Full Time Community Health Worker to educate and connect patients with identified social determinants of health to available resources and assist with navigating barriers. We are looking for candidates who are passionate about delivering high-quality, patient-centered care.

This is a fully onsite in clinic position.
