Job summary

South Side Healthy Community Organization (SSHCO) is seeking a CHW Supervisor to provide leadership for a team delivering community health services across Chicago’s South Side. The role includes staff supervision, health education, and case management with occasional home visits and travel to multiple sites.

The ideal candidate has at least three years in public health, strong leadership and communication skills, and is committed to health equity.

Description

The CHW Supervisor plays a vital role in building SSHCO's capacity to deliver impactful community health services. The position provides comprehensive support to our team of Community Health Workers (CHWs) and Community Health Outreach Workers (CHOWs) by providing direction, supervision, mentorship, and assessment of staff training needs. The Supervisor works collaboratively to guide implementation and quality assurance of all SSHCO affiliated CHW/CHOW interventions. The Supervisor also maintains a reduced caseload of patients and provides health education, health system navigation support, and case management services. The position oversees staff at various locations within SSHCO and requires travel to sites on the South Side of Chicago.

ESSENTIAL JOB FUNCTIONS
  • Supervise and mentor CHW/CHOWs in educating and disseminating health messages in the community, ensuring adherence to protocols and procedures.
  • Guide CHW/CHOWs in connecting community members to health and social services and navigating case management challenges.
  • Lead CHW/CHOWs in recruitment, outreach, and providing ongoing care to community members. This includes onsite visits, home visits, and phone consultations with patients, families, and medical/social service staff. May also manage a caseload of participants.
  • Collaborate with project leaders and other team members to assess intervention effectiveness, monitor and/or assign CHW caseloads, and discuss workloads.
  • Participate in the hiring process for CHW/CHOWs, conduct regular evaluations for direct reports and help CHW/CHOWs establish professional boundaries.
  • Foster a positive teamwork environment. This includes recognizing staff achievements and building strong relationships.
  • Ensure CHW/CHOWs are well‑equipped to succeed by managing workloads, documenting their efforts, and overseeing program‑specific paperwork (activity logs, case notes, evaluation tools).
  • Help CHW/CHOWs develop various business skills such as writing, public speaking, record keeping, leadership, and time management. Identifies and plans continuing education opportunities and assists with the facilitation of training (classroom, on‑the‑job, shadowing, and quality improvement initiatives).
  • Facilitate effective communication between team members, project staff, community partners, and clients. This may involve working with multi‑disciplinary teams in health settings to develop and maintain strong relationships with clinical teams.
  • Assist with organizing data for program evaluation and ensuring the fidelity of data collected by CHW/CHOWs.
  • Actively participates in team meetings and/or external partner meetings.
  • Perform other duties as assigned.

QUALIFICATIONS
  • Flexible work schedule required. Requires availability weekdays (8:00 AM - 4:30 PM) with occasional evenings and weekends as needed.
  • Travel: This position requires travel throughout the Chicagoland area. A valid driver's license, reliable vehicle, and proof of insurance required.
  • Education: High school diploma or the equivalent required. College degree preferred.
  • Experience: At least three years of professional experience in a public health, social service or healthcare industry complemented with a bachelor's degree or equivalent combination of experience and education required.
  • Licenses and Certifications:
    • Up-to-date vaccinations as defined by the CDC, including influenza and COVID-19 required. Proof required prior to the start date.
    • CHW Certification preferred.
  • Computer Skills:
    • Strong proficiency with an EMR system (e.g., Athena, Epic, Cerner) preferred.
    • Working knowledge of Microsoft Office Suite (Word, Access, Excel, PowerPoint).
  • Language Skills: Bilingualism in Spanish if a plus and may be required for some positions.
  • Additional:
    • Familiarity with and/or experience working in Chicago's West/South Side communities (residents preferred).
    • Working knowledge of medical terminology.
    • Proven ability to lead and collaborate effectively with paraprofessional staff.
    • Excellent interpersonal skills with conflict resolution and clear verbal and written communication skills.
    • Strong understanding of the CHW model and experience working with CHWs or within healthcare (strongly preferred).
    • Commitment to racial and health equity.
    • Passion for working with community members and providers.
    • Culturally competent with empathetic mindset.
    • Strong leadership skills in team building and program administration.
