Job summary

Christian Community Health Center is seeking a Community Health Worker (CHW) - Contractor to join our frontline care team serving Southside Chicago communities.

You will conduct home visits, community outreach, and point-of-care screenings (A1c, glucose, blood pressure), document in the EHR, support appointment scheduling, and connect patients with SDOH resources to close care gaps and improve engagement.

Qualifications

• Familiarity with Southside Chicago neighborhoods and cultural dynamics.
• 1+ year of community outreach, patient engagement, or care coordination experience.
• Ability to conduct health screenings and follow basic clinical protocols.

The Community Health Worker (CHW) - Contractor is a trusted frontline member of the FQHC care team, responsible for community outreach, point-of-care testing (A1c, glucose, blood pressure), scheduling support, and connecting patients to social determinants of health (SDOH) resources. The CHW works directly with Southside Chicago residents to close care gaps, support chronic disease management, and improve patient engagement.

• May require walking, lifting up to 20 lbs., and working outdoors.
