Petaluma Health Center in Southern Sonoma County and West Marin is seeking a Team Nurse, RN for a full-time position. You will assess patient problems, implement plans of care, educate patients on health maintenance, and coordinate with Providers to support community health needs.

Qualified candidates hold a California RN license with 1–2 years of community nursing experience. The role includes mentoring staff and participating in ongoing training, with a focus on patient-centered care.

Qualifikationen

• Two year nursing degree required; BSN preferred.
• Current California RN license required.
• 1-2 years of nursing experience, community nursing preferred.

Aufgaben

• Assess patient problems and needs; implement care plans under Provider direction.
• Provide patient education on health maintenance and disease prevention.
• Perform medication reconciliation and manage chronic disease per protocol.
• Mentor staff and lead trainings; assist with team trainings and orientation.
• Collaborate with Care team and Providers to enhance PHC patient experience.
