Job summary

21st Century Home Health Services (21HHS) is seeking a Registered Nurse Case Manager, Level 2, to lead and oversee home health care in the Petaluma, CA area. The RN will create and implement care plans, assess patients, and coordinate with an interdisciplinary team to improve outcomes.

The candidate will supervise two nurses, perform Start of Care, evaluations, and discharge planning, and mentor staff while delivering high-quality home health services in patients’ homes.

Qualifications

• CA RN license in good standing.
• BLS/CPR certification current.
• Experience in home health case management preferred.
• Ability to supervise and mentor nurses.

Responsibilities

• Create and execute the plan of care to promote wellness and prevent re-hospitalization.
• Oversee two nurses responsible for executing the care plan.
• Conduct Start of Care, Evaluations, Resumption of Care and discharge planning.
• Provide wound care, meds education, and home care guidance to patients and families.
• Prepare clinical notes, orders and charting in a timely manner.
• Collaborate with physicians and interdisciplinary team.
