Job summary

Petaluma Health Center is seeking a Team Nurse (RN) to assess patient problems, implement plans of care, and advise on health maintenance under the Provider's direction. You will collaborate with the care team to enhance the patient experience and mentor staff.

The role includes triage for adults, geriatrics, pediatrics, and special cases, plus medication reconciliation, patient education, and chronic disease management per protocol. CA RN license and BLS required.

Qualifications

• Two year nursing degree required.
• Bachelor's or Master's degree preferred.
• Equivalent to an RN or higher degree from an accredited school of nursing.
• One to two years of increasingly responsible nursing experience including community nursing preferred.

Responsibilities

• Assess patient problems and needs; implement plans of care under Provider direction.
• Mentor staff and assist with case management per protocol.
• Provide comprehensive triage and advice (phone and walk-in) for multiple patient groups.
• Process medication refills and educate patients on chronic disease management.

Education

Two year nursing degree required

Bachelor's or Master's degree preferred
