Job summary

Qualifications

• Current active Illinois RN license.
• Graduate of an accredited nursing program.
• Outpatient/primary care experience preferred.

Responsibilities

• Assess patients and monitor health status.
• Document vital signs, medical histories, and other clinical information.
• Provide nursing care according to established protocols and provider direction.
• Administer medications, immunizations, and treatments as ordered.
• Assist providers with examinations, procedures, and follow-up care.
• Provide patient education regarding medications and treatment plans, preventive care, and chronic disease management.
• Coordinate patient care and communicate with providers and other members of the healthcare team.
• Respond to patient questions and clinical concerns.
• Maintain accurate, timely, and confidential documentation in the electronic health record.
• Support infection-control, quality, and patient-safety practices.
• Connect patients with appropriate follow-up care and community resources.
• Maintain compliance with applicable clinical, organizational, and regulatory requirements.
