Axis Community Health is a nonprofit clinic in Pleasanton, CA, delivering comprehensive primary care, mental health, and dental services to diverse communities. This RN role supports underserved populations in an FQHC setting with patient-centered and culturally competent care.

The RN collaborates with providers, medical assistants, and the care team to triage, educate, and manage chronic conditions, while adhering to HIPAA and quality standards.

Formación

• Graduate of an accredited nursing program; ADN required, BSN preferred.
• Minimum of one year experience in hospital or clinic settings.
• Current California RN license; knowledge of primary care services.
• Ability to comply with CPR, First Aid and mandated programs.
• Strong communication and teamwork within an interdisciplinary care team.

Responsabilidades

• Provide direct patient care including medication administration and immunizations.
• Administer triage, patient assessment, and education; coordinate referrals.
• Maintain HIPAA-compliant records and charting in EHR systems.
• Support care team with care planning, documentation, and follow-up.
• Ensure compliance with clinical protocols, safety, and regulatory requirements.
• Participate in staff meetings and ongoing training; adapt to fast-paced settings.
