Job summary

Zufall Health is an organization dedicated to community health in New Jersey. We are seeking a licensed Registered Nurse to provide comprehensive primary care across our clinic, mobile health van, and community settings in collaboration with physicians, nurses, and support staff.

You will perform triage, educate patients, manage labs and vaccinations, document in EMR, and participate in quality improvement. This role requires NJ RN licensure and strong team coordination.

Qualifications

• Knowledge of nursing principles and practices.
• Knowledge of primary care and women’s health issues in an ambulatory care setting and reproductive health/family planning.

Responsibilities

• Assist in patient care in collaboration with care teams and staff.
• Perform triage in clinic or by phone and document interventions in EMR.
• Implement and update policies and procedures per center guidelines.
• Oversee lab services and point-of-care testing per CLIA/CLIS.
• Coordinate vaccines and serve as NJIIS/VFC liaison; organize trainings.
• Educate patients on primary care, vaccines, and testing procedures.
• Coordinate appointments and follow-up care across providers.
