Job summary

Union Community Care is hiring a Community Health Resource Coordinator II in Lancaster, PA. This role uses social work expertise to help patients meet social and behavioral goals, coordinating with medical, dental, and behavioral health teams to manage complex needs.

Responsibilities include applying for public benefits, enrolling in health insurance, and serving as patient advocate across community providers. Fluency in English is required.

Qualifications

• Bachelor’s degree in Social Work, Human Services or related field.
• 3+ years of relevant experience.
• Certified Application Counselor or Navigator, required.
• Must demonstrate effective communication in person and in writing, and ability to work with community organizations, health center staff, and clinicians.
• Ability to document and schedule in the Electronic Health Record.

Responsibilities

• Collaborate with individuals and families to create and supplement care plans based on trust and cultural humility.
• Assist patients with applying for public benefits including Medical Assistance, CHIP, Marketplace and other services.
• Coordinate with medical, dental and behavioral health to serve as a social services case manager.
• Enroll and assist in health insurance enrollment for adults and children.
• Represent Union Community Care at coalition and community meetings and advocate for patients.
