Job summary

Osage Nation Health Center is seeking a Community Health Representative (CHR) to connect Osage and Native clients with essential health resources. The CHR will transport clients to appointments, prioritize Wah-Zha-Zhe Health Center visits, and support disease prevention in the Osage community.

The role requires a CNA certificate, strong communication skills, and the ability to travel and lift up to 50 lbs. Compliance with HIPAA and Osage Nation policies is essential.

Qualifications

• High school diploma or GED required.
• Certified Nursing Assistant (CNA) certificate.
• Must be able to stand, walk, bend, kneel, lift 50 lbs.

Responsibilities

• Serve as liaison between medical providers and clients within Osage Nation Health Center.
• Provide transportation to Osage/Native clients to appointments with priority to Wah-Zha-Zhe Health Center referrals.
• Promote health and wellness and assist with disease and injury prevention in the Osage community.

The Community Health Representative (CHR) acts as liaison between medical provider and client to bridge gaps between Osage/Native clients and health resources. The CHR provides transportation to Osage/Native clients within the Osage Reservation who have no other means of transportation with priority to Wah-Zha-Zhe Health Center appointments and their referred appointments. The CHR strives to improve overall health and wellness and promotes disease and injury prevention of our Osage/Native community members.
