**Job Title: Community Health Representative (CHR)****Location:** Health & Human Services Building**Reports to:** Community Health Nurse**Location:** Health & Human Services Building**Status:** Non-Exempt/Hourly **Open Date: September 18, 2026****Close Date: Open Until Filled****SUMMARY**The Community Health Representative (CHR) services as a community-based healthcare liaison between Tribal Citizens and the Health and Human Services (HHS) Department. Working under the direction of the Community Health Nurse, the CHR supports Citizens in accessing healthcare, human services, and community resources; assists with care plan activities, health education, wellness, and disease prevention; and responds to the health and social needs of individuals and families.The CHR builds trusting relationships and promotes a culturally responsive approach to care grounded in the HHS CEDAR framework: Connected, Engaged, Deep-Rooted, Aware, and Responsive. The CHR is **Connected** to Citizens, families, HHS programs, and community partners; **Engaged** in supporting individuals in achieving improved health and well-being; **Deep-Rooted** in Tribal culture, community, and traditions; **Aware** of the health, social, cultural, and environmental factors that influence wellness; and **Responsive** to the needs, referrals, and changing circumstances of the community.This position requires a strong foundation in healthcare and the ability to apply medical knowledge and skills while providing compassionate, respectful, and culturally appropriate support to Citizens across the lifespan.**DUTIES AND RESPONSIBILITIES** include, but are not limited to, the following:* Under the direction of the Community Health Nurse + Make home visits within the Tribal community, with emphasis on the elderly and those with chronic health needs, to assess health/human services need and carry out care plan activities including assistance with activities of daily living and instrumental activities of daily living. + Make visits to community members in hospitals or other institutions, as needed + Transport clients to medical, behavioral health, and specialty related appointments/services when deemed appropriate + Assist in planning and implementing outreach/health promotion disease prevention activities including community events + Identify community needs and make recommendations for program improvement and/or development + Train to implement the Injury Prevention Program, with the assistance of current outside resources + Provide health education and patient reference materials to clients to assist them in understanding and increasing positive health practices/outcomes + Assist with planning and implantation of grant related activities + Assist with the organization and implementation of specialty clinics and general clinics when necessary + Respond to referrals from Tribal Programs and community members + Refer clients to other health/or human service resources, as needed + Increase the wellness of Citizens and staff through the maintenance of Wellness Center equipment and memberships. + Serve as a patient advocate by supporting Citizens during healthcare encounters, participating in interdisciplinary care coordination meetings and case conferences, facilitating communication between patients, families, providers, and community resources to promote culturally responsive, person-centered care.* Serve at all times as a Goodwill Ambassador from the Tribe* Develop and maintain working relationships with outside agencies/programs in order to access those resources* Represent the Tribe’s position if requested, in meetings with other Tribes or with funding agencies* Recognize and be sensitive to the cultural diversity inherent in the practice of self-care/health/wellness within Native American communities and educate appropriately* Data entry and accurate Electronic Health Record documentation* Other duties as assigned**SKILLS AND ABILITIES*** Previous experience in Tribal governments* Working knowledge of Indian culture and traditions* Knowledge of primary healthcare, preventative health, medical terminology, childcare, nutrition and/or first aid* Ability to work with all age groups* Must be able to maintain confidentiality according to Tribal, State, Federal and HIPPA guidelines* Ability to read, analyze and interpret professional journals, technical procedures, and governmental regulations* Must possess a strong work ethic including being service orientated* Must have excellent interpersonal, organizational and time management skills* Possess effective oral and written communication skills* Must be self-motivated* Must have a high level of accuracy and attention to detail* Ability to manage multiple tasks and adjust to changing priorities in a professional manner* Excellent computer skills, including proficiency in the use of Microsoft Office programs* Possess the ability to operate standard office equipment* Ability to work in a team-orientated environment and maintain an effective team working relationship with co-workers* This position will be cross trained to be utilized in other HHS programs (Clinic/Business Office)**C****ON****DITIONS OF EMPLOYMENT*** Occasional non-traditional hours including working evenings, weekends and holidays as required to meet program needs and deadlines* Ability and willingness to be on call as needed* Ability and willingness to travel, locally and out of state, for program requirements, meetings, and continuing education* Must possess and maintain a valid driver’s license, reliable transportation and be insurable* Must maintain applicable certifications/licenses including CHR and BLS Certification throughout employment**SECURITY SENSITIVE**This position will have access to information that is security-sensitive and thereby subject to additional provisions.
