Qualifications

• Associate's Degree in Community Health or related field required; additional public health training is a plus.
• Experience building and sustaining partnerships across healthcare providers and community partners is required.
• Familiarity with care coordination workflows and closed-loop referral systems is preferred.
• Ability to travel between system facilities and off-site locations as needed.

Responsibilities

• Serve as the primary spokesperson and sustain engagement with providers and community partners.
• Convene Hub members to assess local needs, set priorities, and develop a tailored Hub strategy.
• Manage localized program implementation, budgets, and coordination of delivery partners.
• Monitor Hub progress and performance and escalate as needed.
• Oversee navigator utilization, workflows, referrals, outreach, and documentation.
• Facilitate Leadership Board governance, including agendas and action-item follow-up.
• Coordinate onboarding and alignment across hospitals, public health, EMS, schools, and community orgs.
• Develop and implement referral pathways, escalation protocols, and operational workflows.
• Track and report operational data using dashboards and approved reporting tools.
• Identify barriers and coordinate improvement activities.
• Maintain a visible local presence through stakeholder meetings and community events.
• Support implementation readiness, partner onboarding, and launch preparation.
• Perform other duties as assigned.
