Yavapai Regional Medical Center in Prescott, AZ seeks a Program Manager, Community Health, to lead strategic initiatives that improve community health outcomes and foster wellness, targeting diabetes, obesity, and preventive care.

You will coordinate, implement, and evaluate health education resources, provide one-on-one coaching, and collaborate with clinical teams to ensure messaging and care are culturally and linguistically appropriate while advancing health equity.

Qualifications

• High School Diploma/GED
• Certificate Other Community Health Worker
• Two years of management/leadership experience
• Knowledge of community health, population health, public health principles, and health promotion strategies.

Responsabilités

• Lead and supervise a team of Community Health Workers, providing guidance, coaching, performance management, and development.
• Plan, implement, evaluate, and continuously improve community-based initiatives that promote health equity and outcomes.
• Collaborate with multidisciplinary teams, including clinical providers, behavioral health, care management, quality improvement, and community partners.
• Engage with community groups and individuals to address social drivers of health and ensure culturally responsive services.
