Job summary

Douglas County is seeking a lead for the Community Health Worker program, coordinating, directing and managing operations to meet health department goals and grant requirements.

You will supervise CHWs, coordinate trainings, record activities, and build relationships with clients, county staff, agencies and the public.

This role requires a public health/management background, strong communication, and commitment to confidentiality and professional standards.

Qualifications

• Bachelor’s degree in Public Health, Public Health Administration, Health Care Administration, Social Work, or Management required (see ad).
• Two years as a Community Health Worker or experience in community health outreach preferred.
• One year of supervisory experience preferred.
• Two years of MS Office experience required.
• Valid driver’s license and own transportation required.
• Pre-employment criminal record check and drug screen required.
• Equivalent combination of education and work experience may be substituted.

Responsibilities

• Coordinate and monitor program activities to meet health department goals and grant requirements.
• Coordinate, record, and maintain training activities for CHWs.
• Establish and maintain work relationships with clients, county employees, officials, attorneys, agencies and the public.
• Ensure compliance with Civil Service Rules, policies, and laws to promote a safe, respectful environment.
• Facilitate training and mentoring for CHWs on core competencies and career development.
• Coordinate staff development needs and opportunities for CHWs with the Supervisor.
• Arrange logistics for community health education activities and outreach events.
• Collaborate on Social Determinants of Health (SDOH) screening and connect clients to non-medical resources.
• Draft strategies to help clients navigate healthcare systems and advocate for health needs.
• Oversee CHW documentation, analyze outreach metrics and report program impact.
• Prepare records, reports, and correspondence; maintain knowledge via seminars and trainings.
• Respond to inquiries within expertise and update standard procedures and policies.
