Job summary

The Lawrence County Health Department is seeking a Support Services Associate/Community Health Worker (CHW) to provide administrative support for health programs and assist with community outreach and education.

The role emphasizes accurate recordkeeping, customer service, and collaboration with community partners to enhance clinic and public health services. Training and guidance will be provided to help support health initiatives in the community.

Qualifications

• Provides administrative support for health department programs.
• Assists with community outreach, education, and program coordination.

Responsibilities

• Coordinate assigned community health programs, projects, coalition meetings, and related activities.
• Maintain collaborative relationships with community partners.
• Represent the health department on interagency councils, committees, and community meetings as assigned.
• Develop and implement program policies and procedures to ensure compliance, accountability, and program effectiveness.
• Prepare statistical reports on program performance, needs, services, information and demographic data.
• Provide information and interpretation of the assigned program project to all staff and general public.
• Route and direct the public to the right and correct information.
• Maintain medical records and files.
• Enter information from the Patient Encounter Forms (PEF) Outcome or Assessment Information Set (OASIS).
• Generate and issue Woman Infant and Children (WIC) vouchers.
