Job summary

Douglas County Nebraska is seeking an experienced Community Health Worker Program Supervisor to coordinate, train and support CHWs in meeting health department goals and grant requirements.

You will build relationships with clients, staff and partners, ensure compliance, and drive program improvements through data and outreach activities. Strong leadership and confidentiality are required.

Qualifications

• Bachelor’s degree in Public Health, Public Health Administration, Health Care Administration, Social Work, or Management.
• Two years as a Community Health Worker or in community health outreach preferred.
• One year of supervisory experience preferred.
• Two years of MS Office experience required.
• Valid driver’s license and own transportation required.
• Pre-employment criminal record check and drug screen required.
• Equivalent combination of education and work experience may substitute for requirements.

Responsibilities

• Coordinate and monitor program activities to meet health department goals and grant requirements.
• Coordinate, record, and maintain training activities for CHWs.
• Establish and maintain work relationships with clients, County staff, officials, attorneys, law enforcement and the public.
• Comply with Civil Service Rules, policies, and laws to ensure a safe, respectful work environment.
• Facilitate training and peer mentoring for CHW staff on core competencies and career development.
• Coordinate staff development needs and opportunities for CHWs.
• Coordinate logistics for community health education activities and outreach events.
• Collaborate on SDOH screening tools and link clients to non-medical resources.
• Draft strategies to help clients navigate healthcare systems and advocate for health needs.
• Oversee CHW documentation, analyze outreach metrics and report program impact.
• Prepare records, reports, and correspondence; maintain job knowledge.
• Respond to questions within expertise and draft standardized procedures for CHW workflows.
• Demonstrate empathy and respect toward community values; ensure program readiness.
• Maintain confidentiality of medical and personal health information.
• Provide guidance to subcontractors and staff; promote positive work environment.
• Participate in internal and community committees; assist in emergency planning and response.
• Operate a county vehicle as required; maintain regular attendance.
