IMPaCT Community Health Provider Partnership

We seek warm-hearted primary care physicians (MD/DOs) to provide general supervision for Community Health Workers working in your local community.

If you work with us, you will:

1. Dedicate blocks of time to conduct real-time telehealth visits with patients outreached by Community Health Workers. For example, you might do a telehealth visit in your pajamas on a Tuesday afternoon with a recent widow at a senior center or a new immigrant at her church. The Community Health Worker will brief you on the patient's background, allowing you to evaluate their needs and determine eligibility for Community Health Integration services, especially if social needs are affecting their medical condition.
2. Review notes from Community Health Workers integrated into your practice's EHR, describing social support, advocacy, and navigation services provided to patients. You will also submit brief monthly notes and bills for services rendered.
3. If patients lack a trusting primary care provider, you are welcome to serve as their doctor. Community Health Workers will assist patients in finding suitable primary care providers. You will respond to urgent clinical messages from Community Health Workers regarding new patients before they establish primary care elsewhere.
4. Collaborate with a team of Community Health Workers who will refer new patients to you. You can earn $100 per hour plus additional revenue to cover practice costs. Revenue is also available for E/M visits addressing urgent issues for new patients bridging to primary care. The estimated time commitment is 5-10 hours per week.
5. Your practice will handle billing and collections. IMPaCT Care will ensure payment to local organizations employing Community Health Workers. Limited access to your EHR will be needed for scheduling, clinical messaging, and billing reports.
6. Must be licensed in Indiana.
