Community Health Physician Partner

IMPaCT Community Health Provider Partnership

We seek warm-hearted primary care MDs/DOs to supervise Community Health Workers working in your local community.

If you join us, you will:

1. Dedicate blocks of time to conduct real-time telehealth visits with patients outreached by Community Health Workers, such as a recent widow at a senior center or a new immigrant at her church. You will receive a quick hand-off from the Community Health Worker about the patient as a person, then evaluate their needs to determine eligibility for Community Health Integration services—addressing medical conditions affected by social needs.
2. Review notes in your practice’s EHR, integrated with our Community Health Worker documentation app, describing social support, advocacy, and navigation provided to patients. You will also submit brief monthly notes and bills for services rendered.
3. If patients lack a primary care provider, you are welcome to be their doctor, though Community Health Workers will assist in finding a suitable primary care home. You will respond to urgent clinical messages from Community Health Workers regarding new patients.
4. Work with Community Health Workers who will refer new patients to you. You can earn $100 per hour, with additional revenue covering practice costs. You will also earn for any E/M visits addressing urgent issues for new patients. The estimated workload is 5-10 hours per week.
5. Your practice will handle billing and collections. IMPaCT Care will pay the local organizations employing Community Health Workers. Limited access to your EHR will be required for scheduling, messaging, and billing reports.
6. Must be licensed in Indiana.
