Job summary

Fair Haven Community Health Care in New Haven, CT is seeking a Pharmacist to deliver medication and pharmaceutical care to our underserved community, including immunizations and MTM. You will work with a collaborative FHCHC team, ensure safe dispensing, manage MTM for high-risk patients, counsel on chronic conditions such as diabetes, and help maintain 340B contract pharmacy compliance.

Qualifications

• Bachelor’s degree in pharmacy required, PharmD preferred.
• Related pharmacy experience required.
• Pharmacist registration in the State of Connecticut is required.
• Medication Therapy Management (MTM) certification or willingness to obtain within first year of employment.
• Immunization certification is preferred or willing to become a certified immunizer within 90 days of hire.
• 340B experience preferred.
• The successful candidate must demonstrate high quality interpersonal relations and customer service skills.

Responsibilities

• Dispense medications and ensure safe and efficient delivery to all customers.
• Consult with customers and providers to maintain open lines of communication.
• Facilitate Medication Therapy Management (MTM) for FHCHC patients.
• Consult with patients regarding chronic conditions such as diabetes to improve health outcomes.
• In absence of the Pharmacist in Charge, oversee pharmacy technicians and store operations.
• Ensure pharmacy is compliant with 340B contract pharmacies.
• Provide professional consultation to FHCHC patients and prescribers.
• Direct technicians' workflow and verify orders, charges, and inspections, manage inventory and medication packaging.
