Main Purpose of Job

The main purpose of this position is to perform at the full performance level of work providing health education, disease prevention information, SDOH screenings and resource navigation to medically underserved populations within the community. Operating within the Outreach and Education Division, this position supports the agency's mission by promoting healthy behaviors, reducing health disparities, and bridging gaps between vulnerable community members and critical public health services.

MINIMUM QUALIFICATIONS
  • Education: Graduation from an accredited high school or possession of a high school equivalency certificate.
  • Experience: One year of experience in providing information on health care and disease prevention within the community.
  • Notes:
  • 1. Candidates may substitute experience providing assistance to individuals in a health care or social services setting on a year-for-year basis for the required education.
  • 2. Candidates may substitute U.S. Armed Forces military service experience as a non-commissioned officer in Health Services classifications or Health Services and specialty codes in the health related field of work on a year-for-year basis for the required experience.
DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should possess:

  • An active Certified Community Health Worker (CCHW) credential in the State of Maryland
  • Experience linking residents to local resources.
LICENSES, REGISTRATIONS AND CERTIFICATIONS

Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator's license valid in the State of Maryland.
