Job summary

Blue Ridge Health is seeking a Community Health Worker to join our Community Outreach Team. The CHW will connect individuals to health care services, provide basic health education, and assist with resource navigation and event planning. Occasional evenings and weekends may be required.

Applicants should have a CHW certification (or obtain within 1 year), a passion for community engagement, and strong communication and organizational skills. Proficiency with Microsoft Office and EMR is expected.

Qualifications

• CHW certification required; if not certified, must obtain within 1 year of employment.
• Lived experience or prior work with homeless populations is preferred.
• Ability to communicate clearly and professionally to facilitate collaboration.
• Strong organizational skills and the ability to work autonomously.
• Proficiency with Microsoft Office programs and Electronic Medical Records.

Responsibilities

• Connect individuals to health care services.
• Provide basic health education/coaching to the community to prevent negative health outcomes.
• Provide resource navigation assistance to meet individual’s needs.
• Help plan, organize and staff community health education and screening events; occasional evenings and weekends may be required.
• Take basic vital signs and participate in basic health screenings.
