Blue Ridge Health is seeking a Community Health Worker to join our Community Outreach Team. You will serve as a liaison between BRH and the communities we serve, helping to increase access to health care and community resources in the communities we serve so as to improve health outcomes.

Responsibilities include connecting individuals to health care services, providing basic health education and coaching, and coordinating resource navigation.

Qualificações

• CHW certification required. If not certified, must obtain certification within 1 year of employment.
• Passion for community engagement work and serving diverse populations.
• Strong communication to collaborate internally and externally.
• Reliable, flexible, and able to manage competing priorities.
• Ability to work autonomously with good organizational skills.

Responsabilidades

• Connects individuals to health care services.
• Provides basic health education/coaching to the community.
• Provides resource navigation assistance to meet needs.
• Plan, organize and staff community health education and screening events.
• Takes basic vital signs and participates in health screenings.
• Occasional evening and weekend work may be required.
