Job summary

Neighborhood House in Seattle seeks a Community Health Nurse to promote family wellbeing across Head Start and Medicaid long-term care programs. You will assess needs, plan care, and deliver nursing services and case management for families.

This role spans multiple sites including Head Start locations in Seattle, Burien, Tukwila, with home visits across King County and requires WA RN licensure and strong communication skills.

Qualifications

• Bachelor's or higher degree in Nursing or RN with WA license.
• Three years’ experience in nursing or community health.
• Knowledge of well-childcare, prenatal, postpartum care, and communicable diseases.
• Knowledge of or willingness to learn geriatric and/or nursing care for persons with disabilities, wound management, health promotion, and case management in public health and home settings.
• Comfort with public speaking and providing training to non-medically licensed staff.
• Training and experience with public health, or health education.
• Experience with data collection, analysis, and reporting to stakeholders including parents, staff, program management and program leadership.

Responsibilities

• Plan, implement, and evaluate health services and programs across Head Start and ECS departments.
• Review health records and determine up-to-date preventive and primary care; assist families with scheduling.
• Provide nursing consultations and case management to Medicaid long term care clients.
• Lead Health Services Advisory Committee and train staff on health screenings and inclusions of children with health needs.
• Travel to multiple Neighborhood House sites and conduct home visits within King County.
