Job summary

Merakey Total Health in Philadelphia, PA is seeking a Nurse Practitioner to deliver comprehensive primary care in our Mt. Airy community-based clinic. You will work with physicians and an interdisciplinary team to provide patient-centered care across the lifespan.

The ideal candidate has 1–3 years NP experience, MSN, PA license, and a commitment to integrated, community-focused care. This position follows a Monday–Friday schedule with collaboration and quality improvement.

Qualifications

1–3 years Nurse Practitioner experience, preferred in Primary Care.
Experience working with patients with behavioral health or intellectual/developmental disabilities preferred.
Experience working in a community healthcare setting is preferred.
Experience with telehealth is a preferred.

Responsibilities

Clinical Assessment & Patient Care: conduct assessments, histories, exams; diagnose and manage conditions; prescribe medications; develop treatment plans.
Diagnostic Evaluation & Treatment Planning: order and interpret tests; create evidence-based plans.
Patient & Family Education: educate on diagnoses, treatment, medications, preventive care.
Documentation & Compliance: maintain EHR documentation; ensure regulatory standards.
Care Coordination & Collaboration: coordinate with physicians and multidisciplinary team.
Leadership, Quality & Community Engagement: support quality improvement and partnerships.
