Job summary

Northwestern Medicine is seeking a Community Health Worker I to bridge the gap between the health system and the community in Chicago. The role emphasizes outreach, education, and connecting individuals to essential services while maintaining regulatory compliance.

The CHW collaborates with Population Health, Primary Care, and FQHCs to improve access and social determinants of health outcomes, documenting activities in the EMR and supporting family education.

Qualifications

• Minimum two years of customer service experience in a professional office environment.
• Ability to communicate effectively with patients and the community.

Responsibilities

• Act as liaison between health system, social services and the community.
• Provides telephonic support to patients to assist them in achieving social stability and care continuity.
• Supports individuals, families, and communities to access NM’s social drivers resources.
• Facilitates patient and family education promoting social wellness and health system knowledge.
• Collaborates with Population Health, Primary Care, Community Affairs and FQHCs.
• Completes social drivers of health screenings and provides resources.
• Follows regulatory reporting requirements.
• Documents data in the electronic medical record to track needs and interventions.
• Uses motivational interviewing and conflict resolution to resolve issues.
