Job summary

Rush University Medical Center in Chicago seeks a Community Health Worker to operationalize Rush’s social determinant of health and chronic disease strategy. The CHW will liaise with community members, patients, and partners, conducting screenings, providing navigation services, and offering self-management support in clinic and community settings.

Qualifications

• High school graduate or GED.
• 1-2 years of experience working in community settings as a CHW or navigator desirable.
• Ability to work independently with strong multitasking, organization, communication and information systems experience (Microsoft Office).
• Knowledge/experience with care-management related functions (interviews, discharge planning, referrals).
• Comfort working in diverse settings and across socio-economic levels, especially Chicago’s West Side.

Responsibilities

• Provide health education to help clients achieve health goals.
• Create tailored strategies to address community health concerns.
• Increase health knowledge and self-sufficiency through outreach and counseling.
• Address barriers to care and connect clients to resources.
• Provide direct services such as blood pressure monitoring and health screenings.
