Job summary

Engaging Solutions, LLC in Indiana is seeking a Community Health Worker to serve as the primary contact for members enrolled in the Indiana PathWays for Aging program. You will conduct face-to-face and telephonic health screenings, assess needs, and referrals to community resources while maintaining confidentiality.

The role emphasizes outreach, coordination with case management, and support for timely access to care from home when not on-site.

Qualifications

• High School Diploma or GED required.
• Up to 3 years of experience in health and human services.
• Ability to obtain Indiana driver's license and transport.
• Bilingual candidates preferred.
• Knowledge of HIPAA and confidentiality essential.

Responsibilities

• Coordinate follow-up care plan needs for Members by scheduling appointments or enrolling in programs.
• Support member access to care, including telephonic and in-person visits.
• Make referrals to Case Management Services.
• Assess adherence to medical treatment plans via phone or home visits.
• Identify barriers to plan compliance and coordinate resolutions with the team.
• Assist in accessing community resources (education, housing, transportation, recovery supports).
• Coordinate identification and referral to local, state or federally funded programs.
• Participate in cross-functional teams on projects and process improvements.
• Maintain confidentiality of sensitive information and ensure quality outcomes.
