Job summary

Christiana Care Health System is seeking a Community Health Worker to serve as a trusted link between patients, care teams, and community resources. You will help individuals navigate healthcare, address social needs, and reach personal health goals through outreach and ongoing support.

This full-time, community-based role covers New Castle County with occasional travel to Kent/Sussex Counties, including home visits and clinic/community settings.

Qualifications

• High school diploma or GED is required.
• 1–2 years in community outreach, healthcare support, social services, or related field.
• Strong communication, relationship-building, organizational, and problem-solving skills.
• Ability to work independently and with healthcare professionals and community partners.
• Valid driver's license and access to a reliable personal vehicle.

Responsibilities

• Build trusting relationships with patients through outreach, individualized support, and follow-up.
• Help patients identify health goals and overcome barriers to care.
• Connect patients with healthcare services, community programs, and social resources.
• Coordinate patient-centered support with healthcare teams and community organizations.
• Engage with patients in homes, ChristianaCare facilities, and community settings while managing a caseload.
